System disk creation

I attempted to make a new system disk from my DOS 3.3 disk and I'm having issues getting it to boot, can someone tell me if I'm doing it right:

Boot to DOS 3.3
format /s b:
swap disks, etc.
disk "b:" has command.com on it
copied config.sys, and othe drivers from a: to b:

Boot to b: gets "Disk Boot failure".

Did I do this wrong, or do I have a bad disk?
Doesn't format report bad sectors?
Any form of chkdsk I can do? lol

Okay, found out apparently I don't have DOS 3.3, the disk was labeled incorrectly.

So on a related note, I have DOS 2.1 and the files for DOS 3.3 (on a 3 1/4" disk), however, I can't for the life of me figure out how to get a new system disk made on a 5 1/4" DD disk. I can't run sys.com on DOS 3.3 because "Incorrect DOS version", and I can't seem to find away around this. I've tried various things to get around it but I seem to be stuck.

Does anyone know how I can get a new DOS 3.3 system disk made on a 5 1/4" disk when I can't boot it in the first place?

Sorry - I've been distracted with the PCjr telnet server test for the last two days. When things get back to normal I'll get the DOS patches posted and start paying attention here again.
